What does a clinically integrated network offer patients?

As a member of one of our clinically integrated networks, you will have access to a care coordinator. A care manager is a registered nurse or social worker who will work closely with you to develop a customized approach to your specific health care needs.

It’s a care coordinator's responsibility to help educate you on the care process and address medical and any other questions you or your family might have. They will be there to support you with appointment scheduling assistance and other gaps in your wellness plan.

Our care coordination model helps you navigate the healthcare system to more easily make appointments, see the provider you need to see, and schedule diagnostic and lab tests, etc. Your care coordinator oversees your spectrum of care over time. In addition, chronic disease management is aided through weekly calls, home visits, and other activities.

Wellness services we offer just for you

Health coaching for a healthy you

Health coaching is a highly personalized service with the goal of improving your health by working on healthy lifestyle behaviors, like diet and exercise. As a patient, you will have the opportunity to work one-on-one with a health coach to develop a personalized wellness plan that fits your specific health needs.

Your health coach will help you set and achieve wellness goals and provide you with the resources you need to live a healthier life. We work with patients who are overweight, have diabetes, high blood pressure, high cholesterol, and other chronic diseases.

You will meet face-to-face with a health coach in your provider's clinic. After the first session, your health coach will follow up with you weekly, biweekly, or monthly. These follow-up sessions can take place over the phone or in person, depending on your preference.
